The present invention relates to a shoulder hanging massage apparatus for providing a massage to a shoulder and a neck of a user by an embedded massage ball rotating and moving back and forth while a massage apparatus hangs on a shoulder of the user. To this end, the shoulder hanging massage apparatus according to the present invention includes: a drive unit including an upper case and a lower case, which are coupled to each other to form a hollow space inside thereof, a drive motor provided on a center of the hollow space, a worm gear formed on a front and a rear part of a rotating shaft of the drive motor, and a driven gear engaged to the worm gear； a forward and backward motion unit including a reciprocating rotation shaft having a lower part fixedly inserted into an upper part of a rotation shaft of the driven gear in such a manner that the reciprocating rotation shaft moves forward and backward, a round plated shaped disk cam fixed to an upper surface of the upper case and having a cam hole formed at a center part thereof, through which the reciprocating rotation shaft passes and a drive surface on an upper surface of the round plated shaped disk cam to allow a roller to roll, and the roller fixed to the reciprocating rotation shaft to roll on a top of the drive surface； a massage unit including a rotating plate fixed to an upper end of the reciprocating rotation shaft and a massage ball protruding through an upper surface of the rotating plate； an outer skin including a device accommodating unit to embed the drive unit, the forward and backward motion unit, and the massage unit and a wrist gripping unit protruding downward through each end of the device accommodating unit in a longitudinal direction； and a controller including a PCB box provided inside the lower case and an operating panel provided on a surface of the outer skin.
