The present invention relates to: a composition, which is for preventing or treating pulmonary fibrosis and includes an exosome extracted from an adipose-derived stem cell as an active component； a use of the exosome in the preparation of a pharmaceutical for preventing or treating pulmonary fibrosis； and a pulmonary fibrosis treatment method including a step for administering a subject with a pharmaceutical composition including the exosome as an active component. An exosome extracted from an adipose-derived stem cell according to the present invention includes growth factors, such as HGF (hepatocyte growth factor) and IL-10, related to pulmonary fibrosis treatment, and can significantly reduce the fibrotic region in pulmonary fibrosis-induced test animal models, and thus, can be usefully used in the prevention or treatment of pulmonary fibrosis. Moreover, in a pulmonary fibrosis treatment using an exosome according to the present invention, there is little concern of side effects from steroid-based drugs and antioxidants used in typical pulmonary fibrosis treatments occuring, and exosomes can be naturally transported to the lungs through injection- or ingestion-based administration, without surgical treatment, and thus, pulmonary fibrosis can be treated simply and economically.La présente invention concerne : une composition destinée à prévenir ou traiter la fibrose pulmonaire et qui comprend comme principe actif un exosome issu de cellules souches dérivées du tissu adipeux ； une utilisation de l'exosome dans la préparation d'un médicament pour la prévention ou le traitement de la fibrose pulmonaire ； et une méthode de traitement de la fibrose pulmonaire comprenant une étape d'administration au patient d'une composition pharmaceutique comprenant l'exosome comme principe actif.
