In particular, the present invention relates to an apparatus and method for monitoring patient movement during irradiation in connection with cancer treatment in a radiotherapy apparatus. A patient marker is attached to the patient's nose and images of the patient marker and the fiducial reference marker are taken at predetermined time intervals. The reference marker is located at a position that is defined relative to a patient fixation configuration for fixing the patient during treatment. Based on the image taken by the optical tracking device, the relative position of the patient sign relative to the reference sign is determined.
