A barrel adapter (220) for a syringe (200) having a barrel (218) and plunger (212). The adapter comprises a barrel tip (232) and a needle retraction mechanism (211) having a needle subassembly (221) with a needle (201) and a lumen, actuator subassembly (210), and biasing member (240). The needle subassembly moves from an injection to a retracted position within the barrel tip. The actuator subassembly includes a retainer (224) connected to the barrel tip, a stylet (254) held in a stylet disc (214), and a prong (242) passing into the retainer. The stylet slides within the needle lumen and is spaced from the needle tip in the injection position. The biasing member biases the NOM from the retainer. A locking arrangement compresses the biasing member when locked and releases the biasing member when actuated by depression of the plunger, which moves the prong against the retainer.
