An inhalable glucocorticoid composition for use in the therapy of a patient suffering from severe and uncontrolled asthma, wherein said composition is administered as a nebulized aerosol generated by an inhalation device, said device being adapted to: (a) emit the nebulized aerosol during the inhalation phase of the patient at a rate not exceeding approximately 20 liters per minute； (b) emit, by inhalation phase, a total volume of at least about 0.4 liters of gas phase, including said gas phase, nebulized aerosol and optionally air without aerosol and (c) emit, per inhalation phase, no more of approximately 150 milliliters of air without aerosol before nebulized aerosol is emitted, provided that the composition does not comprise methylxanthine.Una composición de glucocorticoide inhalable para uso en la terapia de un paciente que padece asma grave e incontrolada, en donde dicha composición se administra como un aerosol nebulizado generado mediante un dispositivo de inhalación, estando adaptado dicho dispositivo para: (a) emitir el aerosol nebulizado durante la fase de inhalación del paciente a una velocidad no mayor que aproximadamente 20 litros por minuto； (b) emitir, por fase de inhalación, un volumen total de al menos aproximadamente aproximadamente 0,4 litros de fase gaseosa, incluyendo dicha fase gaseosa aerosol nebulizado y opcionalmente aire sin aerosol y (c) emitir, por fase de inhalación, no más de aproximadamente 150 mililitros de aire sin aerosol antes de que se emita aerosol nebulizado, siempre que la composición no comprenda metilxantina.
