A device comprising a speculum 30 (preferably an otoscope or ear speculum) and an attachment means (40/45 figure 1) for attaching a camera 20 to the speculum. The speculum has a first end 32 with a first opening, and a second end 36 with a second opening. The attachment means is for attaching or is attached to the first end of the speculum so that the camera has a first position substantially in the plane of the first opening such that it can view a region beyond the second opening. The camera may be displaced, possibly by pivoting or rotating from the first position to a second position in which it does not extend or only extends partly across the first opening. When in the second position the camera is preferably not positioned to view along a central axis of the speculum between a first and second opening. The device may be configured to allow an instrument to reach through the first opening. The attachment means is preferably a ring which fits around the first end of the speculum and the camera is preferably secured to the attachment portion by a hinge so that the camera can move.
