A method for calculating the index of microcirculatory resistance based on myocardial blood flow and a CT image, comprising: extracting a myocardial image to determine a myocardial volume; positioning a coronary ostium and accurately segmenting a coronary artery; generating a mesh model required by the calculation by means of edge detection of coronary artery data; determining resting myocardial blood flow and CFR by means of non-invasive measurement; calculating total flow at an inlet of the coronary artery under a maximum hyperemia state; determining the flow under the maximum hyperemia state of different blood tubes in a coronary artery tree to determine a flow rate V1 under the maximum hyperemia state; obtaining a transmit mean time Tmn under the maximum hyperemia state, taking V1 as the flow rate at the inlet of the coronary artery, and calculating a pressure drop ΔP from the inlet of the coronary artery to the narrow distal end of the coronary artery, wherein the average pressure Pd in the coronary artery at the distal end satisfy Pd=Pa-ΔP; and calculating the index of microcirculatory resistance.
