PROBLEM TO BE SOLVED: To make it possible to extract highly dislocated lumbar disk herniation which has been difficult to extract by a conventional percutaneous endoscopic instrument.SOLUTION: An instrument according to the present invention includes a jaw part in a tip side of an elongated shaft insertable into a percutaneous endoscope system. The jaw part is rotated in a plain perpendicular to a jaw mouth side direction using a pivot shaft parallel to the jaw mouth side direction as a fulcrum, and the jaw mouth is brought into contact with dislocated disk herniation. Next, the whole instrument is turned to hold the disk herniation between the jaw part and a vertebral body bone surface, and the jaw part is restored to a state before the rotation to pull the disk herniation. The disk herniation is moved to a range in which forceps can be used, thereby making it possible to extract even highly dislocated disk herniation with the forceps.
