A pressure monitoring device is provided for a tool, such as a medical endoscope. The device has a housing fluidly coupled to the interior volume of the endoscope. A pump is used to change the pressure within the device housing and endoscope, thereby creating a pressure differential from the ambient environment. During the endoscopic procedure, the pressure within the device housing is monitored, and changes in pressure above predetermined limits produce a warning signal indicating that a leak has occurred within the endoscope. The device housing may be directly attached to the endoscope and located remotely and may be integrated with an accessory device that is connected to the coupling tube or connected to the endoscope. The pump may be integrated inside the device housing or it may be a separate external pump coupled to the housing. The pressure difference may be positive or negative with respect to the surroundings.
