PROBLEM TO BE SOLVED: To provide an information storage device in which an information storage element is built in a dental material used in an oral cavity, thereby enabling a wearer to live without any fear of risk that might be caused by longterm use of the device.SOLUTION: The information storage device can be formed in an artificial tooth 10, which is an example of a dental material used in the oral cavity of the wearer, the artificial tooth 10 comprising a recess 11 formed on a side of an occlusion surface S thereof. A filling resin material is filled in the recess 11 to form a filling part 12. An information storage element 13, from which information that specifies the wearer is electrically read by an information reader, is buried in a state being sealed by a sealing part 14 made of a sealing resin material in the filling part 12.
