A foldable sleeve for retaining a surgical instrument is presented. Thefoldable sleeveincludes a first panel having a first set of lock tabs, a second panelhingedly connected to the firstpanel, the second panel having a first pair of openings and a second pair ofopenings, and a thirdpanel hingedly connected to the second panel, the third panel having a secondset of lock tabs.The first panel and the third panel are each selectively movable over aportion of the secondpanel to retain the surgical instrument within the foldable sleeve.
