PROBLEM TO BE SOLVED: To provide a chemical mixed injection instrument in a new structure, which allows puncture to a rubber stopper of a puncture needle in a state where a port part of a vial is accurately positioned so as to spread in an axis perpendicular direction to the puncture needle.SOLUTION: A chemical mixed injection instrument includes: a positioning means for positioning and holding a flange-like port part 92 of a vial 86 so as to spread in the axis perpendicular direction of a vial guide member 14 and a slide movement blocking means for blocking axial direction inward movement in which the vial guide member 14 approaches a partition wall 18 and then holding the vial 86 in the state of being isolated from a puncture needle 24 until the port part 92 of the vial 86 is positioned and held by the vial guide member 14.
