A removable attachment for an insulin injector pen 7 has a plurality of tapering pads 9 that, when pressed against the skin, flex and grip the skin because of their dome-like arrangement. The pads each has a plurality of protrusions, or bumps that enhance the grip on the users skin. The radial and axial movement of the pads pinches the skin around the site of the injection to ensure that the dosage is delivered to the appropriate layer of subcutaneous tissue. The device may have a conical part (22, fig 5) within the tapering pads that guides the needle and protects the users fingers from the needle. In another embodiment the device is held by a gimbal (25, fig 6) that is in turn held by a handle (26, fig 5).
