PROBLEM TO BE SOLVED: To provide an ophthalmic laser treatment apparatus suppressing energy loss of laser light while suppressing complication of an irradiation optical system, and allowing acquisition of the laser light having a proper beam profile.SOLUTION: The ophthalmic laser treatment apparatus including the irradiation optical system for forming the laser light in a spot size and irradiating the laser light on the eye of a patient includes: a multi-mode fiber connected to the incident end of the irradiation optical system a light guide optical system guiding the laser light to the multi-mode fiber, and having a single mode fiber and a lens forming a Gaussian beam emitted from the single mode fiber in a prescribed beam diameter on the incident end of the multi-mode fiber and a holding member holding a curved state of the multi-mode fiber in a prescribed shape. The multi-mode fiber has a length set such that the beam profile in the emission end becomes a desired shape.
