PROBLEM TO BE SOLVED: To provide a user-friendly information collection device in which button selection is facilitated.SOLUTION: The information collection device comprises: a plurality of selection buttons 6a-6e and 7a-7c which can be pressed by a user； a communication module 10 capable of storing at least identification information on the user and information on the selection button pressed by the user, and wirelessly transmitting the stored information to a host device； and a cancellation button 4 for cancelling the selection of the information on the selection button pressed by the user. The communication module 10 includes an IC chip 12, and a button cell 14 supplying power source voltage to the IC chip. The IC chip 12 includes a storage unit 21 storing information specifying the pressed selection button and information on a time at which the button is pressed in time series, a control unit 22 controlling storage of information to the storage unit, and a radio communication unit 23 performing proximity radio communication with the host device.
