An aerosol delivery system comprises an aerosol generator 230 for producing an aerosol, a continuous positive pressure airway ventilator 300 for producing a pressurized ventilation gas, a splitter 312 for splitting the pressurized ventilation gas into a carrier gas 316 and a ventilation gas 317, a conduit 302 from the ventilator to the splitter and an aerosol transition adapter 100 arranged to combine the aerosol produced by the aerosol generator with the carrier gas from the splitter. The transition adapter divides the carrier gas into a plurality of streams of carrier gas, which are directed to at least partially encircle and to flow in parallel with the aerosol entering the transition adapter, and which form an entrained aerosol 240. An aerosol delivery connector 330 has a port 334 for receiving the entrained aerosol, a port 332 for entry of the ventilation gas, an interface port 336 for delivering the entrained aerosol from the aerosol transition adapter and the ventilation gas from the splitter to a patient through a patient interface 340.
