PROBLEM TO BE SOLVED: To provide a data book for pets to be written by pet guardians.SOLUTION: The data book in which information of pets is written and read among pet guardians, veterinarians, or foster keepers, includes: a first summary provided with entry columns of a name, date of birth, breed, male/female, vaccination history, and precautions； and a second summary provided with columns for entering symptoms, a state of injury, treatment thereof, and instruction items from veterinarians. The pet data book further includes a third summary on which pet guardians can write about memories.【課題】 飼い主により記入可能なペット用データ帳を提供すること。【解決手段】 ペットの飼い主、獣医あるいは里親との間でやり取りされるペットの情報を記載したペット用データ帳であって、名前、生年月日、種類、雌雄、予防接種履歴、注意事項の記入欄を設けた第１一覧表と、病状、怪我の状況、その処置、獣医からの指示事項を記載する欄を設けた第２一覧表を備えたペット用データ帳を提供する。ペット用データ帳は、更に飼い主が思い出などを記載できる第３一覧表を備えることができる。【選択図】図２
