Since each of the site I sodium channel blockers have a unique activity andcannot be used toextrapolate the same effective dosage for another site I sodium channelblocker, studies were conducted toidentify dosages of neosaxitoxin ("NeoSTX") and bupivacaine, alone or incombination with epinephrine,to provide two to three days of pain relief in humans. Bupivacaine-NeoSTXcombinations produce morereliable blockade and longer duration blockade compared to NeoSTX alone. Thethree-way combinationof NeoSTX-bupivacaine-epinephrine produces more prolonged local anesthesiathan the two-waycombination of NeoSTX- bupivacaine. Addition of epinephrine to this NeoSTX-bupivacaine combinationdramatically prolongs the duration of complete blockade to a mechanicalstimulus. These results led todevelopment of specific combination dosage formulations.
