Systems and methods are provided that are suitable for extracorporeal lung assistance that exposes blood to dialysis fluid across a semipermeable membrane. The dialysate is characterized by a buffer and has a high buffer capacity for H + ions. The dialysate can be recycled and used repeatedly, the pH can be adjusted and other fluids added. Certain substances may be removed from the blood and the amount of these substances removed from the blood may be monitored or quantified substantially automatically or substantially continuously.
