The surgical guiding instrument has two gripper heads formed at a distal end (26) of the surgical guiding instrument. The gripper heads are designed for a gripper movement relative to each other. A guide rail (33) is provided which defines a guide axis (36) for a cutter. The guide rail is pivotable around a pivot axis (34) so that the guide axis overstretches a work sector with the pivot movement around the pivot axis. The work sector cuts an imaginary connecting line between the gripper heads. An independent claim is also included for a method for processing of facet joint extension on vertebral body.
