Devices and methods for treating patients suffering from complete or quasi-complete obstruction are provided. The device is equipped with a support catheter for transoccluded procedures. The support catheter may include an elongated body that extends between the proximal and distal ends. The elongated body may have lumens for receiving the guide wires and, in some cases, for supporting the guide wires. The support catheter may include a transverse tip located on the distal portion of the support catheter. The transverse tip may have a ring structure that is fixedly attached to the elongated body. The transverse tip may include a plurality of teeth disposed on the distal surface of the transverse tip.
