A needle shaped therapeutic instrument alignment system for use with ultrasound guided procedures such as percutaneous infiltration, biopsy, cryo and thermal ablation. The system comprises an alignment board 5h that is detachably mounted to an ultrasound probe 5a or shield 5b of an ultrasound probe, and a slider upon which a needle shaped therapeutic instrument such as a syringe is detachable clamped, wherein the slider 5g is movably mounted to the alignment board, and manually moved by a physician. The device assists in stabilising the ultrasound probe and aligning at least the tip of the needle 5f with the ultrasonic beam from the ultrasound probe. The planular surface of the board may be mounted or angulated, parallel, or perpendicular to the sonic plane of the ultrasound probe. The board may have marks or tracks to guide the direction and insertion depth of the needle tip. Alternatively the board has means to track detectable means e.g. magnets, conductors in the slider and therefore needle orientation and/or position as it enters a body. The board or slider may have an adjustable bubble level to maintain the inclination of the board / needle.
