The assembly has a tibial phantom of a tibial implant adapted for attachment to tibia (T) of a patient. An aiming guide is set into an instrumentation element through a talus (A) and a calcaneus (C) of the patient along an axis for implanting a talo-calcaneal anchoring keel. The tibial phantom and the aiming guide are mechanically connected with each other to restrict movement of the tibial phantom and the aiming guide relative to each other along vertical and medio-lateral directions, where the aiming guide is guided in placement of the instrumentation element. An independent claim is also included for a surgical method for positioning an ankle prosthesis.Afin de poser une prothèse de cheville incluant un implant tibial et un implant astragalien pourvu d'une quille d'ancrage astragalo-calcanéen, l'invention propose un ensemble d'instrumentation chirurgicale comportant :- un fantôme tibial (60) de l'implant tibial, adapté pour être fixé au tibia (T) d'un patient, et- un guide de visée (80), adapté pour mettre en place un élément d'instrumentation (90) à travers l'astragale (A) et le calcanéum (C) du patient, selon un axe (Z-Z) d'implantation de la quille d'ancrage astragalo-calcanéen,le fantôme tibial et le guide de visée étant conçus pour coopérer mécaniquement l'un avec l'autre de manière à, alors que le fantôme tibial est fixé au tibia, assembler le guide de visée au fantôme tibial, en fixant, à des jeux fonctionnels près, leur positionnement relatif suivant des directions verticale et médio-latérale, puis utiliser ce guide de visée pour mettre en place l'élément d'instrumentation.
