To provide an electrocardiographic waveform timing detection device and a medical image diagnostic apparatus that reliably keep track of an R wave, which serves as a trigger in determining a time phase of a heart, in an electrocardiographic waveform to thereby decrease misdetections.SOLUTION: An electrocardiographic waveform timing detection device according to the embodiment comprises an absolute value circuit, a threshold determination circuit, and a comparator. The threshold determination circuit comprises: a heart rate calculation circuit for calculating a heart rate using an electrocardiographic waveform obtained by measuring a subject with a biological signal measuring device； a threshold setting circuit for setting a threshold； and a comparison determination circuit for comparing the calculated heart rate with the number of R wave detection triggers detected by using the set threshold to determine whether or not the two match, to thereby determine an R wave detection trigger threshold.
