FIELD: medicine.SUBSTANCE: invention relates to medicine, specifically to ophthalmology. For the surgical treatment of a through macular rupture by a three-port vitrectomy, a flap of the internal border membrane is formed by removing fragments thereof from the macular area with preservation of an intact portion of the membrane around the rupture, dissecting and inversing the flap in the direction from the outer border to the center, the introduction of the perfluororganic compound into the vitreous cavity with subsequent replacement of it and the liquid with air. Wherein the flap of the internal border membrane to cover the rupture is formed by removing five fragments thereof; first two fragments in the form of bands 0.2–0.4 mm wide and 2.0–2.5 mm long are removed from the periphery of the macular area to the center of the rupture, ending at 0.1–0.2 mm from the edge of the rupture, wherein the angle between them open to the temporal side of the eye is 70–80°; second two fragments of the membrane are removed in the opposite direction alternately starting from the distal ends of the first removed fragments and moving along the rupture circle until they are joined; fifth fragment of the membrane is removed from the temporal side by moving along the circle, beginning from the distal ends of the portions formed when the first two fragments of the membrane were removed, thus finally forming the flap of the internal border membrane. Before the moment of dissecting the flap, a perfluororganic compound is introduced into the vitreous cavity. Using an aspiration cannula with a silicone tip, the gel substance is removed in the lumen of the rupture. Edges of the rupture are connected by using vacuum aspiration. Flap is neatly smoothed with a tweezer, eliminating the folds of the membrane along its edge.EFFECT: method reduces the risk of operative and postoperative complications, improves the effectiveness of the treatment of patients with continuous macular rupture.1 cl, 1 ex
