Disclosed is the use of acetyl L-carnitine, or a pharmaceutically acceptable salt thereof, in combination with an antihypertensive drug, for preparing a medicament for the prevention or delay of onset of type 2 diabetes and its clinical complications in pre-diabetic subjects with insulin resistance. Further disclosed is the use of acetyl L-carnitine for preparing a medicament for the reduction of high systolic blood pressure or high pulse blood pressure, in pre-diabetic subjects already in treatment with an anti hypertensive drug.
