PROBLEM TO BE SOLVED: To provide an osteosynthesis plate capable of reliably capturing and fixing bones on both sides of a fractured part in a state where bone dislocation is attempted in opposite directions on both sides of the fractured part. SOLUTION: A first support portion 11 located on a first bone surface 91 side of a bone, and a plate connecting portion 14 located along a bone transition surface 93 that moves in a direction substantially perpendicular to the first bone surface. The second bent surface 92 is bent in a substantially right angle direction from the plate connecting portion, and is in contact with a range of the second bone surface 92 of the bone which is in a front-back relationship with the first bone surface and which is not in a facing relationship with the first support portion 11.
