A dosing unit (1) for an infusion pump device comprises a piston pump with a pump cylinder (2), a plunger (3) arranged within said cylinder, and a plunger driving part (4) provided for transmitting rotational torque from a driving unit to the plunger without itself being linearly displaced. The cylinder, the plunger and the plunger coupling element are coaxially arranged along a longitudinal axis (20) and rotatable around said axis in regard to static parts (12) of the dosing unit. The plunger has a shaft (32) with a thread (33, 33) and the cylinder has a threaded sleeve part (26) with a thread (23, 23). One of the two threads is an outer thread and the other one is an inner thread, said two threads engaging with each other in such a way that a rotational movement of the plunger around the longitudinal axis results in an additional linear displacement of the plunger along said longitudinal axis. The plunger driving part has a driving rod (41) that is arranged in a longitudinal bore (322) of the plunger shaft, the driving rod being linearly displaceable within the longitudinal bore along the longitudinal axis, and being rotationally engaged with the plunger shaft. One or more first coupling parts (78, 79) are mounted to or are integral with the cylinder, and one or more second coupling parts (77) are mounted to or are integral with the plunger driving part and/or the plunger. The first and second coupling parts interact in such a way that the first and second coupling parts are bidi- rectionally switchable between a first state and a second state, by reversing the rotation direc- tion of the plunger driving part the first and second coupling parts are unidirectionally switchable from the first state to the second state, by mechanically blocking cylinder rotation or actuating the first coupling part and the cylinder is rotationally coupled to the plunger driv- ing part in the first state of the first and second coupling parts and not rotationally coupled in the second
