The purpose is to provide a tacrolimus-containing pharmaceutical composition which is a creamy preparation for external application having good feeling upon use and has high stability of a main ingredient contained therein (high main ingredient residual ratio), and allows easy control of skin concentration of the main ingredient. The present invention relates to an oil-in-water type creamy composition comprising (A) tacrolimus, a pharmaceutically acceptable salt thereof, or a pharmaceutically acceptable solvate thereof, (B) an oil prepared by mixing (a) a medium-chain fatty acid triglyceride with (b) ethylene glycolsalicylate and/or diisopropylsebacate, (C) an emulsifying agent having a HLB value of 12 or more, and (D) a hydrophilic polymer, and having a pH value of 4 to 7.사용감이 좋은 크림상의 외용제로서, 제제 중의 주약 안정성(주약 잔존율)이 높고, 피부 중 농도를 용이하게 컨트롤 가능한 타크로리무스 함유 의약 조성물을 제공하는 것을 과제로 한다. 수중유형의 크림상 조성물로서, 상기 조성물이, (A) 타크로리무스, 그 제제학적으로 허용되는 염, 또는 그 제제학적으로 허용되는 용매화물, (B) (a) 중쇄 지방산 트리글리세라이드와 (b) 살리실산 에틸렌글리콜 및/또는 세바스산 디이소프로필을 혼합해서 이루어지는 유제, (C) HLB값 12 이상의 유화제, 및 (D) 친수성 고분자와를 함유하고, pH 값이 4～7인 것을 특징으로 한다.
