PROBLEM TO BE SOLVED: To acquire a high-quality subtraction tomographic image in a radiation image processing device, method, and program.SOLUTION: First and second image acquisition parts 31 and 32 acquire a plurality of first and second projection images taken by tomosynthesis imaging on first and second image-taking conditions respectively. A reconfiguration part 33 reconfigures the first and second projection images using only the processing other than filtering processing in reconfiguration processing including the filtering processing, and generates a plurality of first tomographic images and a plurality of second tomographic images in each of a plurality of tomographic planes of a subject. A subtraction processing part 34 generates a subtraction tomographic image from the first and second tomographic images.
