In the present description, a dose counter is disclosed for counting doses of a drug substance, given or remaining in a metered dose inhaler. The dose counter contains a rotated first gear having a circular arrangement of ratchet teeth, as well as display means associated with the first gear, while the display means has a visible set of dose counting signs established when rotated to a predetermined position in response to rotational movement first gear. The dose counter further comprises an actuator having a first ratchet pawl for engaging the ratchet teeth of the first gear in response to a dose of drug substance, as well as a second ratchet pawl for engaging the ratchet teeth of the toothed wheel associated with the display means.
