A prodrug composition for treating diseases, disorders or conditions mediated by the binding of an opioid to opioid receptors, comprising a conjugate of 3,6-diaspirin-hydromorphone in a pharmaceutically effective amount and a biologically acceptable carrier. The prodrug composition of claim 1, wherein the conjugate is used to treat narcotic or opioid abuse； to prevent narcotic or opioid withdrawal； for the treatment of moderate to acute pain； to reduce or prevent oral, intranasal, or intravenous drug dependence； or for resistance to oral, intranasal, or intravenous drug dependence. 3. A prodrug composition according to claim 1, wherein oral administration of the conjugate provides improved bioavailability and release rate over time compared to unconjugated hydromorphone for the same time period. A prodrug composition according to claim 1, wherein oral administration of the conjugate gives less variability in the oral pharmacokinetic (PK) profile compared to unconjugated hydromorphone. The prodrug composition of claim 1, wherein oral administration of the conjugate causes less side effects compared to unconjugated hydromorphone. The prodrug composition of claim 5, wherein the reduced side effect is reduced opioid-induced constipation.
