A positive airway pressure (PAP) system with a PAP device, a patient circuit with an air delivery conduit with mask and tube interface unit, a controller monitor operable to measure a parameter indicative of a patients condition over a period of time and the controller determines the amount of mask leak and measures the mask leak against at least one predetermined mask leak setpoint threshold. The PAP device is configured to deliver therapeutic treatment pressures based on a reduced calibration being independent of the patient circuit, and vary or change to change the pressure delivered by the fan motor variable speed drive when the monitored parameter indicates a lack of patient improvement, provided that the mask leak is classified as being below the at least one predetermined mask leak value. The sensors include an air pressure and flow rate sensor and noise sensor to measure snore sound. The processor has an automatic calibration learning system.
