The surgical assembly (1) includes: - a bone anchor screw (2) having a portion (2a) for anchoring in the vertebral column, and a head in the form of a bushing (2b) and - an extension device (3) to be placed around the bushing (2b) of the bone anchor screw (2) so as to thereby create a longitudinal extension. Said extension device (3) includes a first tubular element, referred to as an "inner tube" (4), having an inner diameter greater than or equal to the inner diameter of the bushing (2b) of the bone anchor screw (2) and longitudinally extending by means of two diametrically opposite arms (4a), thus forming a clamp capable of clamping onto the outer surface of the bushing (2b) so as to be locked onto the latter. The extension device (3) includes a second tubular element, referred to as an "outer tube" (8), slidably mounted along said inner tube (4).
