The assembly (1) has a blocking unit mounted in a longitudinal boring in a proximal portion (5) of a femoral intramedullary nail (2) and cooperating with an external surface of another proximal portion (17) of a fastening screw (3) in an assembled position. The external surface of the latter proximal portion of the screw comprises grooves (19) distributed on a periphery of the latter proximal portion, and extending parallely to an axis of the screw. The blocking unit cooperates with one of the grooves so as to block the screw in rotation in the assembled position.Cet ensemble de fixation comporte un clou intramédullaire (2) comprenant une portion proximale (5) comportant un trou transversal destiné au passage d'une vis de fixation et un alésage longitudinal débouchant d'une part dans l'extrémité proximale du clou intramédullaire et d'autre part dans le trou transversal, une vis de fixation (3) destinée à être engagée dans le trou transversal ménagé dans la portion proximale (5) du clou intramédullaire, la vis de fixation (4) comportant une portion proximale (17) pourvue, sur sa surface extérieure, d'une pluralité de cannelures (19) réparties sur sa périphérie et une portion distale filetée (18) destinée à être vissée dans la tête fémorale, et un organe de blocage (4) destiné à être monté dans l'alésage longitudinal ménagé dans la portion proximale (5) du clou intramédullaire et agencé pour coopérer, en position montée, avec l'une des cannelures (19) de manière à bloquer en rotation la vis de fixation.
