The invention relates to a device with a positioning frame which can be adhered onto the skin and with a transdermal therapeutic system which can be inserted into a cut-out that passes through the positioning frame, the active substance-dispensing outlet side of the inserted transdermal therapeutic system facing the skin. The device comprises a tool unit which can be inserted into the cut-out of the positioning frame and centered in said cut-out in order to produce openings at least in the uppermost layer of the skin. By means of the invention, the active substance transfer of the transdermal therapeutic system is improved.
