PROBLEM TO BE SOLVED: To provide an alpha-proteinase inhibitor (A1 PI) for delaying the onset or progression of pulmonary exacerbations.SOLUTION: Disclosed herein is a composition comprising an effective amount of an A1 PI, 25-750 mg/day, for delaying the onset or diminishing the progression of a first set of symptom(s) associated with the pulmonary exacerbation in a subject in need thereof by inhalation administration, where the subject is identified by having a second set of symptom(s) as a predictor of pulmonary exacerbation, where the first set includes: (f) shortness of breath； (g) fatigue； (h) chronic productive cough； (i) reduction in forced expiratory volume (FEV)； and (j) decrease in the expiratory gas markers of respiratory inflammation, and the second set includes: v) increased cough； vi) change in sputum； vii) decreased appetite and/or weight loss； viii) change in respiratory examination and/or respiratory rate.SELECTED DRAWING: None【課題】肺の増悪の発現または進行を遅延させるためのアルファ1-プロテイナーゼ阻害剤の提供。【解決手段】必要とする対象に吸入によって投与することにより、前記対象における肺の増悪に関連する1つまたは複数の第一のセットの症状の発現を遅延させるまたはその進行を縮小させるための、1日当たり25mg～750mgのA1PIである、有効量のA1PIを含む組成物であって、前記対象が、肺の増悪の予測指標となる1つまたは複数の第二のセットの症状を有することから同定され、前記第一のセットの症状が、(f)呼吸不足；(g)疲労；(h)慢性の湿性咳；(i)努力呼気量(FEV1)の低下(j)呼吸器炎症の呼気ガスマーカーの低下を含み、前記第二のセットの症状が、v)咳の増加；vi)痰の変化；vii)食欲減退および/または体重減少；viii)呼吸器の検査および/または呼吸数における変化を含む、組成物。【選択図】なし
