The present invention is the identification of a micro RNA (miR-450b) as a non-invasive biomarker in pregnant women infected with Hepatitis E Virus (HEV). This miRNA can be used for prognosis/diagnosis of pregnant women at higher risk of having acute liver failure due to HEV infection.Hepatitis E virus (HEV) infection causes acute viral hepatitis (AVH-E), which is generally a mild and self-limiting disease but can be fatal in the presence of pre-existing liver disease or in pregnancy. Studies have shown that during pregnancy, HEV infection leads to worse obstetric and fetal outcomes than acute viral hepatitis of any other cause. During pregnancy, 25-35% of infected women with HEV develop liver failure with high maternal and fetal morbidity and mortality. About 70-80% HEV infected pregnant women clear the virus spontaneously and the remaining are likely to progress towards severe hepatitis and acute liver failure (ALF). Pathogen activated miR expression profiles are crucial for the initiation of pathogenic events and disease outcomes while gene expression regulated by specific miRs may vary in response to different infections, in pregnancy and liver failure. miR profiling has been used to identify distinctive miR signatures, their gene targets and pathways in pregnant and non- pregnant females suffering from acute self-limiting HEV infection or acute liver failure. The invention is thus to identify a miRNA which can be used to predict the likely outcome of disease (acute liver failure followed by death) in pregnant women infected with HEV. The invention will be used as a diagnostic tool for better health care management of these women leading to reduction in morbidity and mortality.
