The present invention relates to an instrument including a bearing member and an actuator spaced apart from the bearing member in a proximal direction. The instrument includes or may be attached to an elongate member extending from the actuator location toward the bearing member. the elongate member may be attached to or extend from a portion of an implant, the location being spaced from the support member by a distance in the proximal direction.
