An ophthalmic lighting system can include an optical fiber configured to transmit a light beam output by a light source and focused by a collector. The optical fiber can include a proximal portion, a distal portion, and a central portion. The proximal portion may be configured to receive a light beam that is focused by a collector. The distal portion may be configured to emit a light beam to illuminate the surgical field. The central portion can extend between the proximal portion and the distal portion. The proximal core diameter may be greater than the central core diameter and the distal core diameter. The ophthalmic illumination method can include using a concentrator to focus the light beam emitted by the light source onto the proximal portion of the optical fiber.
