PROBLEM TO BE SOLVED: To provide an ophthalmic laser surgery device in which convergence position of a pulse laser beam can be adequately scanned.SOLUTION: An ophthalmic laser surgery device 1 comprises: an XY scan unit 25 a light guide optical element 31 an objective lens 35 and a Z scan unit 44. The XY scan unit 25 includes at least one of deflection devices 26, 27 deflecting pulse laser beam, and scans the pulse laser beam in a direction crossing an optical axis. The light guide optical element 31 arranged in a downstream side of an optical path of the pulse laser beam compared with at least one of the deflection devices 26, 27, includes refractive power to guide the pulse laser beam into the downstream side of the optical path. The objective lens 35 condenses the pulse laser beam via the XY scan unit 25 and the light guide optical element 31 on a tissue of a patient eye E. The Z scan unit 44 changes the light path length between the light guide optical element 31 and the objective lens 35 while the position of the objective lens 35 is fixed.
