The present invention relates to a forming apparatus for a freeform five-shaft polymeric scaffold comprising: a support table which is arranged on a base； a worktable which is arranged to be separated from the support table and allows a polymeric scaffold to be formed； a dispenser device which is arranged on an upper part of the support table and discharges raw materials forming the polymeric scaffold towards the worktable； a dispenser transfer means which transfers the dispenser device to an X axis, an Y axis and a Z axis； and a worktable transfer means which rotates the worktable from side to side and tilts the worktable back and forth.
