A positioning tool 10 has a cannula 14 with a mandrel 16 slidable inside the cannula 14. An expandable structure 17 formed of rods 18 with intermediate pivot points 24 has a pouch 2 temporarily secured thereto by sutures. A proximal end 20 of each rod 18 is pivotally connected to the cannula 14, and a distal end 22 is pivotally connected to the mandrel 16. When the mandrel 16 is drawn proximally back inside the cannula 14, the rod ends 22 approach the rod ends 20 and the structure 17 expands to position the pouch 2 for attachment within a gastrooesophageal junction region.
