A revision hip implant comprises a stem, sleeve, and neck. A proximal IM portion and a distal IM portion of the stem are generally aligned along a long axis of the stem. A body portion of the stem is proximally oriented and has a surface extending generally radially out from the long axis of the stem. The sleeve is configured to be received over the distal intramedullary canal portion of the stem and further has an inner surface configured to mate with the distally extending taper surface of the proximal intramedullary canal portion. The neck is configured to be received in the body portion of the stem. The body portion of the stem has a neck receiving surface configured to mate with a neck taper portion of the neck. The neck taper portion is oriented at an angle relative to the long axis.
