An internet-based disease monitoring system comprising a patient-operated device, a mobile communication device and a remote server. The patient-operated device includes a sensor producing input signals responsive to a patient condition, and a sensor transmission module coupled to the sensor. The sensor transmission module comprises a digital signal processor operatively coupled to at least one processor, the digital signal processor configured for receiving the input signals from the sensor and converting the input signals to digital data; the at least one processor operatively coupled to a transmitter, the at least one processor configured for converting the digital data into a data package, the data package including a device identifier associated with a medical device coupled to the sensor; and the transmitter configured for encrypting the digital data and transmitting the data package to a mobile communication device via SMS for further transmission to a remote server for decryption and analysis of the digital data based at least in part on the device identifier. The mobile communication device is responsive to receipt of the data package from the transmitter, the mobile communication device forwarding the data package to a predetermined network address. The remote server is at the network address and responsive to the data package received from the mobile communication device, the remote server analyzing the data package and producing analysis signals representing the results of the analysis, and transmitting the analysis signals to predetermined recipients associated with the patient-operated device.
