PROBLEM TO BE SOLVED: To provide a medical image processor, a care system and a medical image processing program capable of automatically detecting an unexpected event which has occurred during irradiation in radiotherapy.SOLUTION: A medical image processor has a first image acquisition part, a second image acquisition part, a feature processing part, a calculation part and a determination part. The first image acquisition part acquires a plurality of first images of a subject picked up by an imaging device at different times. The second image acquisition part acquires a second image of the subject picked up by the imaging device at a time different from the times of the first images. The feature processing part obtains a plurality of positions of a first feature of the subject from the plurality of first images and a position of a second feature corresponding to the first feature in the subject from the second image. The calculation part defines a first closed space including the plurality of positions of the first feature.
