The invention relates to an elongate guide element 22 for guiding an implant along a tunnel in a bone and for securing the implant within the tunnel. The guide element comprises a first end 24 and second end 26; a pair of support apertures 42, 44 spaced apart in a direction along a main longitudinal axis 28; and a saddle 46 extending transverse to the longitudinal axis located between the support apertures. The saddle 46 defines a space 54 between the saddle and the height of the element side at the midpoint 56, forming a recess for receiving part of a support element. In use, a support element (3, see figure 10) extends through one of the pair of support apertures, around the saddle and then through the other aperture, for guiding and securing a ligament 1 into a bone tunnel. The guide element is made by metal injection molding.
