Systems and methods are provided for adjusting the basal / bolus ratio in an effective insulin dosing regimen for a subject comprising daily amounts of basal and bolus insulin drugs that define an initial basal / bolus ratio. A first data set comprising the subject's glucose measurement acquisition values is obtained with respective time stamps for each such measurement acquisition value over time. One or more fasting events are identified over time. For each fasting event, a temporal glucose slope is calculated using the glucose measurement acquisitions in the data set within the fasting event period. The glucose gradient of the fasting event is used to determine whether the subject should make the recommended adjustment of the basal / bolus ratio without changing the daily total insulin medication. When it is determined to make the recommended adjustment of the basal / bolus ratio to the subject, the recommended adjustment of the basal / bolus ratio is communicated.
