A crown (50, 150, 250, 350, 550, 650) in the receiver portion(34, 334, 434, 534, 634) of the coupling member (30, 130, 330, 430, 530, 630) isengaged by the plate member (120, 140, 160, 180, 600) to secure the anchor member(70) in position relative to the coupling member (30, 130, 330, 430, 530, 630).
