A respiratory therapy device 100 including a oscillatory resistance device, preferably with a rocker mechanism 12 that opens and closes an opening 10 when a patient exhales into the device to produce an oscillating, vibrating resistance to flow, the device also includes an acoustic device 200 having a membrane 201 exposed on one side to expiratory gas flow such that it generates an audible feedback signal to the user. Preferably the acoustic device is an aperture covered by a membrane 205 in tension. There may be a pressure responsive valve element (300 Fig 3) that oscillates during exhalation and is the source of the air for the membrane. Preferably the acoustic element only produces a noise when the pressure in the apparatus is greater than a level sufficient to provide effective positive expiratory pressure (PEP) therapy.
